The first step to accessing government-funded aged care services is to apply for an aged care assessment. You can apply online through My Aged Care, by calling 1800 200 422, or in person through an Aged Care Specialist Officer at a Services Australia service centre.

TrueCourse Supports Services can help you understand the aged care process and prepare for your assessment so your needs, preferences and goals are clearly communicated. When preparing, it can be helpful to consider:

  • Your current situation and health needs
  • Tasks you may need help with at home
  • Your independence and wellbeing goals
  • Any support you currently receive
  • Services that could help you remain safe and independent
  • Relevant health or supporting information
  • Whether you would like a family member, friend or supporter involved

During the assessment, a trained assessor will discuss your circumstances, what you can manage independently and where additional assistance may help. Assessments are generally conducted in your home, although other arrangements may be available depending on your circumstances.

If you are approved for Support at Home, you will receive information about the services you have been approved for and your support plan. When funding is available, your referral code can be provided to your chosen provider so they can access the relevant assessment information and begin organising your services.

Our team will work with you to match you with someone who suits your needs, personality, preferences and routine. If you ever feel that the match is not right or you would prefer a change, we’ll help make the process as smooth and stress-free as possible.

Your My Aged Care funding is based on an assessment of your individual care needs. If you are approved for the Support at Home program, an aged care assessor will determine the services you are eligible for and assign you an appropriate funding classification.

Your assessment may consider factors such as:

  • Your health and care needs
  • Your ability to manage everyday activities
  • The support you need to remain safe and independent at home
  • Your personal circumstances and wellbeing goals
  • Any short-term, assistive technology or home modification needs

For ongoing Support at Home services, there are eight funding classifications, with the amount of funding you receive based on your assessed level of need. Your funding is generally provided through a quarterly budget, which you and your provider use to arrange approved services that meet your needs.
